Review Contract, Sign and submit w/payment. Include either proof of insurance or payment for insurance.
Important Information
Food Vendors – Health and Fire inspections will begin at 12:00 on Friday 10/2. Please be at your vendor spot and ready.
Food Permits will be issued by the Chatham Health District after completion of paperwork and inspection of food containment apparatus. The following form can be filled out electronically and must be printed and sent to:
