Review Contract, Sign and submit w/payment. Include either proof of insurance or payment for insurance.
Important Information
Food Permits will be issued by the Chatham Health District after completion of paperwork and inspection of food containment apparatus. The following form can be filled out electronically and must be printed and sent to:
